HOW TO REFER
1. Your Clinical Operating System
You can easily refer through your clinical operating system e.g. EMIS / SYSTEM ONE. The referral goes straight through to our secure NHS.net inbox and our team will contact your patient within 3 days and triage.
2. SUBMIT A REFERRAL
Head to Get started and submit a referral on behalf of your patient and our team will contact them and triage.
3. Self-referral
All patients can self-refer through the
Get started button at the top of our website. This will take them through an online health assessment and based on their responses we will contact them and triage onto the most appropriate programme.
ELIGIBILITY CRITERIA
The criteria vary a little depending on the programme (see below), however to join our programmes, your patient must be:
- A resident of East Sussex.
- Aged 16+ (different age requirements apply for smoking cessation & NHS Health Checks).
- Motivated and ready to make a change.
Adult weight management
- BMI over 30 (over 27.5 for individuals from black African, African-Caribbean and Asian backgrounds or ˃25 where the individual has accessed tier 1 support previously but not been successful in achieving a healthier weight);
- They have a BMI over 40 (-co-morbidities) or over 35 (+co-morbidities), unless pre-assessed and referred by their GP.
Exclusions
- Pregnant or breastfeeding.
- Eating disorder.
- Accessed the service within the last 3 months.
- Underlying medical cause for obesity (these patients would benefit from more intensive clinical management than a tier 2 service).
- Significant unmanaged co-morbidity or complex needs as identified by their GP or other health care professional.
Smoking cessation
- Aged 12+
- Smoked tobacco in the last 24 hours.
Exclusions
- Attempted to quit smoking 4 or more times in the last 12 months.
NHS Health Check
- Aged 40-74 years.
Exclusions
- Have previously had a health check in the last 5 years.
- Diagnosed with heart disease, stroke, diabetes, kidney disease, high cholesterol or high blood pressure.
- Taking medication for blood pressure, cholesterol or diabetes.
